What we collect, and what we do with it.

Where this site is up to
This is the site we are building for Oral Health Home. It is not finished yet.
There are no school visits yet. The first ones are still to come.
The sign-up form is real. What you type in it is saved.
What the form asks for
The form asks for these things.
- Your child’s name, birth date and address.
- Your name, your phone and your email.
- Your Medicare card number and the line number for your child.
- A short note about your child’s health.
- Your name, signed, to show you said yes.
Why we ask for each one
We ask for the name and birth date so we know who we see.
We ask for your phone and email so we can reach you.
We ask for the Medicare card so the bill goes to Medicare. It does not go to you.
We ask about health so your child is safe in the chair.
We ask you to sign so we know you said yes.
Your child does not need a Medicare card. Without one we cannot ask Medicare to pay for the check.
Who sees it
Only our dental team sees your form. Your school does not.
The team is Oral Health Home. This site is run for them.
Their staff read the health answers so your child is safe in the chair.
We do not sell your details. We do not pass them to ad firms.
We do not use tracking tools from other firms. There are no ads.
We do not take payments on this site yet. No payment firm holds your card.
Our dental team has its own notes system. We do not send your form to it yet.
We will say so on this page before we start.
We do not know yet which country holds that system.
If it is not Australia we will say so here first.
The brushing chart
The brushing chart keeps your ticks on your own device.
They stay on that device. You can wipe them at any time.
How it is kept
We keep your details in a database in Sydney. That is here in Australia.
The firm that runs it is called Supabase.
The site itself runs on a firm called Vercel. Your form goes through it to reach us.
Your Medicare number is locked. Your health answers are locked too. Locked means they cannot be read if they are stolen.
Only our staff can open them.
We keep a log of each time a staff member reads them.
We send email through a firm called Resend. Its servers are outside Australia.
We send you one email to say your child is in the club. It uses their first name.
We never put a Medicare number in an email. We never put a health answer or a birth date in one.
Teacher accounts
A teacher can make a free account to get the worksheets.
We ask for their name, email, school and state. We ask which year levels they teach, but only if they want to say.
We use it to give them the files and to see where the program is taught.
We send news only to those who tick the box. They can untick it at any time.
A teacher can delete the account at any time. It stops working at once. We take their details out of view.
We keep the stored copy for one more day. Then a job deletes it for good.
No child details are part of this. It is the teacher’s own details only.
Paying for the check
Some families pay for the check themselves.
We never see your card number. A payment firm takes the card and tells us it is paid.
We keep the date, the amount and a payment number.
We keep that for seven years. Tax rules say we must.
We keep it even after we wipe your child’s health details.
The money goes back the same way you paid it.
These words are a draft. Oral Health Home must sign them off.
How long we keep it
We keep your child’s form for four years from the day we see them.
We keep the Medicare card number for four years too.
If we do not see your child, we keep the form for one year.
We keep the card number for one year in that case.
We keep a note from a school or a family for two years.
A teacher account goes after three years with no sign-in.
We keep a record of the email we send for thirteen months.
This is our rule. We can change it.
We set these times in our own settings. This page reads them.
We keep back-up copies of your form.
If we wipe your details, they stay in those copies for a short time.
Ask us, or ask the Commissioner
Do you want to see what we hold about your child? Ask us. Our email is at the foot of every page.
Do you want us to wipe it? Ask us and we will.
Is a detail wrong? Tell us and we will fix it. We keep a note of what changed and who changed it.
There is a form for all three. Use it and we will email you back.
Are we not able to help? You can then ask the Office of the Australian Information Commissioner. It is free.
