What we hold about your child
You can ask us what we have written down about your child. You can ask us to fix it or to delete it.
What we hold
- Your child's name, birthday and year level.
- Your name, your phone, your email and your address.
- What you told us about your child's teeth and health.
- Your Medicare card number, if you gave us one.
- What you said yes and no to on the form.
What you can ask for
- See it. We write out what we hold and send it to you.
- Fix it. Tell us what is wrong and we change it.
- Delete it. We remove the whole record and it cannot be brought back.
How it works
- Fill in the form below. We send one email to the address you give us.
- Open that email and press the button. Nothing happens until you do.
- A person at Oral Health Home reads it and does the work by hand.
- We write back to the same address.
We answer the same way whether or not we hold anything for your child. Nobody can use this form to find out if a child is on our list.
