What we collect and why
Oral Health Home runs this dental check. This site is run for them.
You can reach Oral Health Home at admin@oralhealthhome.com.
We ask for your Medicare card number. We ask for your child’s line number on the card too.
It is the number beside your child’s name.
They are what lets Medicare pay for the check. Then it is free for you.
Without them we cannot ask Medicare to pay. Your child is still seen, at no cost to you.
Your form goes to Oral Health Home. Your school does not see it.
You can ask to see what we hold. You can ask us to wipe it.
Is a detail wrong? Tell us and we will fix it. We keep a note of what changed.
Are we not able to help? You can then ask the Office of the Australian Information Commissioner. It is free.
