What we ask for
You fill in one form before the visit. Here is what we ask for, and why.
Medicare needs one signed form each year. It lasts until 31 December.
Who can join
Any child at a school or centre we visit can join the club. Your child does not need a Medicare card to join. Every child with a signed form gets a tooth check and a free pack.
What we ask for and why
We ask for your child's name, age and class so we know who to see. We ask about health so we can keep your child safe. We ask for your Medicare card so we can claim the cost. That way you pay nothing.
Who sees it
Only our dental team sees your form. The school does not see it. We keep it locked away. We also use it for research, with your child’s name taken off.
If you are not covered
Some families do not get the free check from Medicare. That is fine. We may call you first. Your child is still seen on the day, at no cost to you.
Other languages
The letter that goes home comes in other languages too. Ask the school office for the one you need.
